Well, it adds interest Another thing, not that they could help this, but you can sometimes see the shadow of the cameraman and his camera as they go down the steps from the red tavern in the eyeshield sequence
I would say that any programme which includes a 'technosorcerer' as a character, is mostly aware of its anachronisms! I find the scene in Series 5, where a Mediaeval monk introduces a 20th Century child to a voice-activated mechanical lift by asking them what an anachronism is, quite memorable!
Ahh! I noticed that anode of power. It was rolled into the force field to release hordriss, and he used his magic to send them to the lift, and the anode was still on the floor (now the lift!)
For example, the first winning team of series 7, Julie. Just before Treguard dismissed them, Majida went to put the sword in the corner and retrieved the mace.
When she returned she accidently knocked over Treguard's battle sword...
Majida went out of view and snigered and Treguard briefly paused before continuing...

In season 5, a team were given a BAG spell to capture Elita's disembodied voice and tried to cast it against a ghost instead. The production crew clearly weren't ready for it though as the letters didn't appear on the screen and there was no effect whatsoever. The team were still allowed to reuse the spell later.
In Series 3 when Cliff was instructed to crawl through a drain tunnel, the production crew could be clearly heard in the background saying, "Get down, right down..."

There was the fire extinguisher as mentioned and sometimes you could make out lines or the Cromakey wall edges. I think I could just about make out where the edge of the floor was where the blocker areas in s5 were. Also similar with the CoB.
There was the incident with Alistair (S4) in The Place of Choice, where he seemed to step off the beem and you could hear it.
Series 3 when Chris' team (I think) got to Merlins throne room, after the three steps, Merlin appeared I think before the lightning.

Sometimes a blocker would pause before sliding completely out into a corridor or a causeway tile would freeze for a moment before plummeting all the way into the chasm beneath it.
Also in the eyeshield sequence of a dungeoneer leaving the tavern, the shadow "they" cast was often a lot bigger than them and it was carrying a box-like object in it's right hand....
As the new team head towards the dungeon gate the camera dipped up (probably as the cameraman stepped down from a step) and showed part of the studio ceiling.
As Brother Strange / Lord Fear in disguise (not sure which) appeared he appeared from the middle of nowhere. The blue screen he was hiding behind was in the middle of the passage and not by a dungeon column.
