Yes I was the team captain for series 4 team 8. It is indeed amazing to think we're still thinking about those episodes 25 years or so later, given that the Internet wasn't around when they were made.

I think that was me (episode 15). As I recall the Adventurer's Code didn't specifically state that we were not able to say 'I challenge you' from series 4, in the event that we got all three questions correct.

Possibly they repeated part of the COB to make it longer on the programme than it was in filming, wombstar. Having watched Team 1 and Team 6 again, my best guess is that under normal circumstances we would have encountered Mogdred immediately after the collapsing bridge scenes, and had to use the TINY spell. Thanks, Evelyn! The team who failed to complete the Block and Tackle in this episode (we met them before and afterwards) did seem to have their dungeoneer continuing to walk forwards towards the wrong exit when the floor fell away.
