An actor switching from one character to another unintentionally - Paul Valentine using Motley's voice when IC as Hands in series 5. It's not a deliberate one-off, but I can't think of another instance of an actor doing that...

Cliff Barry gets Lissard & Raptor slightly confused while he's under the troll, but to be fair he has to switch rapidly from one to the other!

While these can be subjective, I've also noticed Hands slipping into or at least towards Motley in Quest 2 of Series 7 (the scene with Romahna), and vice versa in Quest 1 of Series 8 (the noise he makes upon entering the trapdoor room).

Regarding the original post the swinging pendulum room indeed only appeared once. The room with the green monster made several appearances, once when Cedric attacked it.

Mildred appearing as Treguard and attempting to goad the dungeoneer into the cooking pot. Treguard was put under a spell but the team avoided the trap.

The Medusa room and the dungeoneer/team turning to stone

One team failing to take a guiding rod and trapping themselves because each exit lead back to the room they were in

I can only remember the giant making one appearance (not to be confused with the giant troll)

Gretel being trapped by Ariadne?

A team dying because of darkness (first ever quest)

Moghdred posing as Cedric

Mildred killing one dungeoneer by leading them into a bomb room due to them previously taking the wrong object

Also did the Oracle of Confusion only appear once?

I saw that on Challenge a few weeks ago for the first time and thought that was as creepy as anything. That would have freaked me out as a kid for sure.

Team 12 in season 2 also died when the light source faded away.

Not exactly. It appeared twice, but was played by two different actresses. Audrey Jenkinson played it in season 2, but she left the series at the end of that year, so when the Oracle reappeared in the third season, it was recast with Zoe Loftin in the role.

I suppose you could say it only appeared once as Audrey, and then only appeared once as Zoe.

yeah, and Mr Grimwold appeared just once as well.
I also recall a 'young grimwold' a few years later.. lol

Mr Grimwold made quite a few appearances in Series 3, but I believe you're right about "young Grimwold" in Series 6 only appearing once.

Yes that was the scene with Julias Scaramonger and Sophia!

I can only remember the time Mr Grimwold killed a dungenner, I can't recall any of the others.. but young grimwold was a nice reference to past.

On second thoughts did Young Grimwold make an appearance in series 7? He left the room when Rothbery said he couldn't do anything for him as he was too ugly! I think it was Nicola's quest that happened.
