Things that should/shouldn't have happened...

Discuss here unfairness regarding teams in Knightmare. For example, if a team should have died but were let off - or vice versa.
I can think of two from the current series (5) airing on Challenge at the moment. I remember one team was carrying poison instead of the green arrow (token of the greenwarden). IMO, they should have died there for having the wrong item. I can just picture Gwendolin saying now "But you're carrying poison! I can't trust you - and how do I know you're not going to poison some other innocent creature! I'm gonna put an arrow in you just to make sure..." Ooh, nasty!
Again, a later team got asked 3 questions by Gwendolin, who clearly said that they needed to get 2/3 to get the password. They only got 1/3, but were carrying the token so got the password anyway. I think it should have been like the earlier series. They didn't get enough questions right, so shouldn't have got the password... and been eaten by the Level 1 Blocker.
I think it was the next series where a team _should_ have died in a very dark dwarf tunnel. Treguard hints "If only you had a lantern" but they still somehow got through!
The team that died on the earth-fire-water causeway I think was unfair. The previous team took all the time in the world to get across the right path. This team were taking the right path, but got killed off for taking a split second too long!
